This will depend on the size of the company. In a smaller company you have to do more. In bigger ones, there might be separate level designers, writers and F2P designers.

I've done tons of levels (more from here, but maybe later ;). This one is from a mini-golf game. The black "skull" is a touch plate with a spring. Hit it and it will throw the ball to the next plate and then to the hole - if you get the speed right!
